How referrals
will work.
Suitability

Who our homes
will be for.

Our homes are designed for adults of working age (18 to 65) who have a primary mental health diagnosis and require 24-hour residential support, but who do not have nursing needs requiring registered nursing staff on site.

We will support
Adults with schizophrenia or psychosis, stabilised on medication
Adults with bipolar disorder requiring structured residential support
Adults with complex depression or anxiety with significant functional impairment
Adults with personality disorder where the primary presenting need is mental health and who have been assessed as appropriate for a non-secure residential setting
Adults in step-down from acute inpatient settings
Adults with a history of self-harm in a stable phase, ready for community transition
Referral Process

How referrals
will work.

Serene will accept referrals from Local Authority Adult Social Care teams, National Health Service community mental health teams, inpatient discharge coordinators, and care coordinators working within our commissioning footprint. The pathway below reflects how our referral process is designed to operate from the point our first home opens.

Initial referral
Submit a referral by email with a brief outline of the individual's current placement, mental health history, and presenting needs. We will confirm receipt within one working day.

Referral review and suitability assessment
Our Registered Manager will review the referral against current capacity and the individual's needs profile. We will confirm suitability and next steps within three working days. We will be clear and direct if a referral falls outside what our homes can safely support.

Pre-placement assessment
For suitable referrals, we will arrange a pre-placement visit involving the individual where possible, their care coordinator, and our Registered Manager. A draft care plan will be prepared prior to admission.

Admission and transition
Admission will be planned in coordination with the discharging team. We will operate a structured transition period with increased key worker contact in the first four weeks. The care plan will be formally reviewed at four weeks and again at three months.

Ongoing review and communication
We will participate in Care Programme Approach reviews, funding reviews, and any safeguarding processes as required. We will maintain open communication with care coordinators and commissioning teams throughout the placement.

Urgent Referrals
For time-critical placements.

Where a referral is time-critical, we operate an urgent pathway alongside our standard process.

What we will do
Review within four hours. Assessment within 24 hours. Confirm suitability within 48 hours. Formal in-home risk review after admission.
What we require
Current risk assessment, diagnosis, medication, and known triggers. No nursing needs on site. Funding authorisation. Named care coordinator remaining active post-discharge.
Serene does not accept direct emergency psychiatric admissions. Individuals in acute crisis should be referred to the appropriate National Health Service acute inpatient pathway.

Our rollout is structured to ensure quality and compliance at every stage. We will notify all registered contacts as each home approaches opening.
2026
Home 1 opens in Greenwich Borough. CQC registration complete. First referrals accepted.
2027
Homes 2 and 3 open in North West Kent and Greenwich Borough. Portfolio grows to 18 beds.
2028
Homes 4 and 5 open. Full portfolio operational: 30 beds across both geographies.
